1          TheMaritimeSafetyCommittee,atitsseventy-fourthsession(30Mayto8June2001),considered the issue of reporting near misses and how to promotea no-blame culture.

 

 

2          TheCommitteenotedthenecessityofencouragingreportingofnearmisses,sothatremedial measures can be taken to avoid recurrences.

 

 

3          In order to promotea no-blame culture, Member Governments areinvited to:

 

 

.1         reviewtheirregulatoryandsafetyregimewithaviewtoencouragingthereportingofnear misses without fear of reprisal or punitive action;

 

 

.2         urgecompaniesoperatingshipsundertheirflagnottopenalizepersonsreportingnear misses; and

 

 

.3         urgecompaniesoperatingshipsundertheirflagstoimplementproceduresbywhichpersons shouldonlyreportnearmissestothedesignatedpersonorpersonsandthedesignated person or persons should only pass on such reports in an anonymous form.
