3.1 Communicationsusingmetricwaves(VHF)or hectometric waves (MF) by a contact radio station2
Thecaptaincontactstheoperatorofanaccessibleradiostationintheareawhereheislocatedand requestsatelemedicaladvicefromtheTMAS,usuallythereferenceserviceforthecontactradiostation,but maybetheshipsreferenceTMAS,particularlyforreasonsoflanguageormonitoringthepatient. The communicationmustberoutedfollowingtheemergencyprocedure,asapriorityandfreeofcharge.
3.2 Communicationsusingdecametricwaves(HF): decametricstationshaveaverylongrange whichallowsanyshiptocontactthemregardlessoftheirpositionatsea. Normally,thereusedtobeanational maritimeradiocommunicationcentreusingdecametricwaves. Becauseofthedevelopmentofnewsatellite telecommunications,thistypeofmaritimeradiocommunications,whichwasthebasisforthefirsttelemedical adviceatsea,islessused. Somenational centreshavecloseddown. However,someshipsarestillonly equippedwithdecametricSSBequipmentforlongdistancecommunications.
Inanyevent,undertheGlobalMaritimeDistressandSafetySystem(GMDSS),globalHFcoverage
isprovidedbyanumberofdesignatedstations(seetheGMDSSMasterPlan).
Itisrecommendedthatagreementsbetweenthecompetentnationalauthoritiesandstationoperators shouldallowtransmissionoftelemedicaladvicetotheship'sreferenceTMASfollowingthefree,priority, emergencyprocedures,recognizedformedicalconsultations.
Maritimeradiocommunicationscentresusing decametricwavescontinuetoprovidearadiotelex serviceusedfortelemedicaladvice,withbothmanualandautomaticprocedures.
3.3 Inmarsatcommunications: thevariousInmarsatsystemsoffertwoabridgedcodes : 32and38, whichcanbeusedformedicalassistanceatseabytelephoneortelex(telexonlyforInmarsat-C):
- code32isusedtoobtainmedicaladvice. Somecoastearthstations(CESs)provideadirectlink withthereferenceTMASforthestationoranearbyhospitalwhenthiscodeisused;and
- code38shouldbeusedwhentheconditionofaninjuredorsickpersononboardashipjustifies medicalassistance(evacuationtoshoreorservicesofadoctoronboard). Thiscodeallowsthe calltoberoutedtotheshore-basedserviceorbodycompetenttodealwiththesituation. Some coastearthstationsprovideadirectlinktotheassociatedrescueco-ordinationcentres(RCCs) whenthiscodeisused.
Itshould,however,beemphasised,thatthecapacitytoreceivecommunicationsviathesetwocodes isnotcompulsoryforCES,althoughmoreandmoreofthemarenowbecomingequippedtodoso.
Mostofthetime,theserviceisfreeofchargefortheship,underanagreementbetweentheCES operatorandtherelevantmedicalauthority.
